By submitting the intake form, you acknowledge and agree that all payments made to Esperanza+Fertility LLC are non-refundable. Once the intake form has been submitted and forwarded to the third-party clinic to initiate the fertility process, no refunds will be issued under any circumstances.

This includes, but is not limited to, changes of mind, scheduling conflicts, or dissatisfaction with services.

Please ensure that you have thoroughly reviewed all information and service details before submitting the form and proceeding with payment.
